Biographical questionnaire issued by Warner Brothers, filled in by Arthur Shields. Shields responses to questions were to be used as a source for Fan Magazines and Newspaper stories. As well as biographical details the questionnaire includes questions such as 'Name your first play', Shields response ' "The Lord Mayor" by Edward McNulty" ' and 'List five of your favorite books and authors' to which Shields responded ' "The Bible", "Shakespeare", "War and Peace" by Leo Tolstoy; "Ulysses" by James Joyce.'
